We explain hospitalization requirements during admission , however if you dont understand you can ask to nurse even after hospitalization.

1. About hospitalization procedure
   Patients who are recieving medical treatment from different hospital please bring a referral letter . Patients who have a history of hospitalization please bring hospital discarge certificate.
   1. All necessary documents will be given to you during admission.
   2. Please fill the application form for admission, and submit to the 1F cashier together with the hospitalization insurance deposit 50000 yen and health insurance card or medical insurance.
2. Properties on hospitalization
   Helath insurance、patient registration card、seal、toiletries,razor, chopstick, spoon, tea cup, tissue paper, slipper, underwear socks etc..
3. About leasing of towels and clothing
   For hospital sanitation and infection control, we lease the towel and clothing. Please write your own necesity and submit it to the nurse center. For inquiry please contact to each nursing center.
4. About diapers
   Use given from our hospital. It is not regular waste. Please understand that this waste is treated as an infectious waste. For more information about the fees please feel free to contact us.
5. About hospital billing
   Hospitalization expense bill is handed out before the 10th of next month. Please give the bill to the 1F cashier. After payment we wil give you the reciept. Please pay the bill within 2 weeks.
   ※As to the traffic accident, it will be handled by automobile liability insurance according to rule. Please ask to reception for more information.
6. Please dont bring a lot of money and valuables and foods.
7. Those who use TV, please buy the prepaid card in front of 2F nursing station.

To make pleasant hospital living, we have some request.

1. Please follow your doctor or nurse instruction in the hospital
2. Smoking is prohibited. Please smoke in only allowed places
3. Alcohol is strictly prohibited. If violate the rule will be forced to leave hospital.
4. Visiting hours
   It is from 15PM to 8PM. Please wait during doctor's hospital round. (nIcluding weekend and holiday)
   Visiting registration form is in the nurse station. Please fill the form.
   Please dont bring infant or many people, it is because of infection prevention purposes.
5. About attendance
   In our hospital we adapted new nursing standart therefore family attendance is not required.
   (However, if you would like to accompany your family member due to age and health condition of the patient, please consult your doctor)
   ※Please consult to the nurse in case of meet in recovery room. During hospitalization parking lot is not available.
   Phone call mediator not possible. Only verbal message. Public phone is available waiting room 1F and 3F. Mobile phone is prohibited.
6. Patients who want special room are subjected to following fees per day. (tax included)
   

   Special room	10,500円～26,250円
   Individual room	6,300円
   2 person room	4,200円
   3 person room	3,150円
   4 person room	2,100円

7. Leaving hospital procedure
   Get the permission of leaving hospital from your doctor and after squaring the account of hospitalization fee come back. If you paid hospitalization insurance deposite please remember to submit the form.
8. About parking
   During hospitalization parking is not allowed.

   It is located in 2F, 3F. It is public so write you name.
2. Rooms
   1. Nurse call system is placed in every bed. Press the button talk when staff respond you.
   2. Please use TV card
   3. Air conditioner can be adjusted by remote control in the entrance of the room.
